Immunizations: Brief Provider Interventions

  • Recommend Vaccination for all eligible people:
    • Influenza
      • Age > or < 65 with chronic medical conditions
      • Children 6-23 months
      • Health care workers, residents of nursing homes/residential care facilities
      • Caregivers & household contacts of children < 6 months
      • Women who will be pregnant during influenza season
    • Pneumococcal Vaccine
      • Recommend for all adults > 65;
      • Anyone >age 2 with chronic health problems such as cardiovascular, renal, liver or sickle cell disease, diabetes, alcoholism, immunosuppresion and certain Native American populations
    • Hepatitis B
      • Recommend for patients ages 0-18 and high risk adults
