Myths and Facts About TB

Myth:
Three negative sputum smears for Acid-fast bacilli exclude active pulmonary TB.
Facts:

- If a patient is a TB suspect, TB is not excluded until sputum cultures are negative and/or another diagnosis is made which explains all the findings.
- Half of all active pulmonary TB in New Mexico is smear-negative.
- Smear-negative patients are potentially infectious.

Myth:
A negative PPD and positive control rule out TB.
Fact:
Patients with active TB may have selective anergy to tuberculin.

Myth:
Positive PPDs in individuals with a history of BCG vaccine can be ascribed to BCG. Fact:
Assessment of a positive skin test is the same whether or not a history of BCG is obtained.
