Late Entries and Addenda

• Entry should be clearly labeled as a late entry or addendum
  
• Entry should not obliterate any entry already in the record
  
• An addendum written after an adverse outcome can be worse than not charting at all
  
• Altered records lead to allegations of fraud and “cover up” and put the credibility of the entire medical record and the providers in jeopardy
