Avoid:

• Biased or judgmental entries
  
• Inappropriate correction of errors
  
• Perpetuation of incorrect information
  
• Arguments/conflicts with other physicians, nurses, students or any negative statements about another party
  
• Documenting information that is unrelated to patient care, such as “incident report filed” or “Risk Management notified”
