  • Medical errors kill 44,000 – 98,000 people in US hospitals each year
  • Deaths from medication errors that take place both in and out of hospitals cause more than 7,000 deaths annually and exceed those from workplace injuries
  • The death rate equates to three jumbo jet crashes every two days
  • Goal is to reduce errors over the next 5 years by 50%
