In-Hospital Transfers

  • Written physician transfer orders are required when:
    • A patient transfers to a different level of care
    • A patient is post-op status
    • A patient’s care is transferred to a different medical service
  • The transferring and receiving teams actively communicate
  • Interim summary documentation is good medical practice
