Pitfalls

  • Poor error analyses lead to poor interventions
  • There are different classes of patient safety interventions, with different relative effectiveness – unfortunately, commonly proposed interventions are often ineffective
  • Measurement of effectiveness of interventions can be difficult
  • Outcomes measurement implementation critical, but also difficult
