“Human Error” Cannot Be Eliminated!

  • Natural part of human activity (especially high-risk activities)
  • Interactions of humans with systems/machines can both cause and prevent errors
  • Automation does not eliminate error – it just changes error
  • Key Issues
    • Optimize human/system interactions to minimize chance of error
    • Facilitate early error recognition/containment of damage
    • Facilitate error recovery
