Early Concept

  • Human errors (and resultant adverse patient outcomes) are the result of performance lapses of individuals
  • “Bad Apples” theory
    • Solution to errors is to focus on outliers
    • Education or punishment of involved individuals
    • In Deming model, all such errors are thus “special cause variation”
