  • All orders (not just medication orders)
  • All medication-related documentation
  • Documentation media to which this applies:
    • Handwritten documentation, pre-printed forms
    • Not to computer-generated forms or displays
  • Expected level of compliance:
    • 100% for pre-printed forms, 95% for handwritten documentation
      • “Three strike”- one strike = one or more “slips” per clinician per record
